Diabetic foot remote monitoring at CHSF: Tatiana Bénard's testimonial

Tatiana Bénard, nurse coordinator of the diabetic foot unit at CHSF, explains how Pixacare secures remote monitoring for her patients and strengthens the link between hospital and community care.

Video testimonial: Tatiana Bénard, nurse coordinator of the diabetic foot unit at CHSF, explains how her team uses Pixacare for remote monitoring.

‍

800 patients followed in the community, 24 diabetes beds, and wounds that keep coming back. At the Centre Hospitalier Sud Francilien (CHSF), a public hospital in Corbeil-Essonnes, south of Paris, Tatiana Bénard, nurse coordinator of the diabetic foot unit, needed to stay in touch with her patients without having them shuttle back and forth between home and hospital. Her solution: Pixacare.

‍

From the TéléPied study to Pixacare

It all started in the field. Together with CERID, a diabetes research center, the team launched the TéléPied study: remote follow-up using photos, with no digital tool and no data security. Results came quickly: fewer hospitalizations, faster wound healing, and deteriorations spotted much earlier. The method worked, but the right tool was still missing. In 2023, coming across the Pixacare booth at a wound care congress changed everything: the team finally found a secure version of what they had been piecing together for years.

“We spotted deteriorations faster, we had fewer hospitalizations, and patients healed faster.”

The hospital-community link drives remote monitoring

A patient is discharged after a hospital stay, a day hospital visit or a consultation? With their consent, they are enrolled in Pixacare remote monitoring. Community nurses send most of the photos day to day (podiatrists and primary care physicians also take part, more occasionally), while the CHSF team keeps an eye on how the wound is progressing. The result: deteriorations caught in time, and readmissions arranged directly in the unit, without going through the emergency department. When there is less cause for concern, simply bringing forward a day hospital visit is enough to keep the situation safe.

“All these patients, with their consent, are enrolled in Pixacare remote monitoring. We've never had a single refusal.”

A secure platform that speeds up specialist opinions

Need an opinion from an infectious disease specialist, a vascular surgeon or an orthopedic surgeon? Pixacare has become the team's comparison tool: photos line up over time, so the wound's progress can be seen at a glance. All images are centralized and secure, and health data security has become an even more sensitive issue since the hospital was hit by a cyberattack.

“We were hit by a cyberattack: today we're really glad to have a secure tool where all our photos are centralized, and we no longer store them anywhere else.”

Adoption beyond the hospital walls

Scientific meetings, word of mouth, positive feedback: community care providers have widely adopted the platform. They now have a diabetic foot specialist close at hand, reachable at the first sign of concern. The result: patients and professionals alike feel reassured.

‍

Tatiana Bénard sums up the impact:

“It has made relations between community care and the hospital so much smoother. It has streamlined patient care within the hospital. It has secured our exchanges: our personal phones stay aside, we don't use them anymore, and our children no longer have access to our wound photos. It's been a really great experience with Pixacare. It makes my daily work as diabetic foot coordinator simpler: I no longer go looking here and there, I get straight to the point. I open Pixacare every morning.”
